Prior to beginning this online application, please make sure you have the following information available:

   1.   Your Social Security Number. By Federal Law, FFS must maintain social security numbers for persons to whom FFS makes disbursement. FFS is authorized to make payments to individuals only, not to corporate entities. The number you provide must correspond to your legal last name.

   2.   Credit Card Information. You will be asked to pay nominal non-refundable administrative service fee as part of the application process. Please have a Visa, MasterCard, American Express or Discover card available. The name and address information on the credit card must match your name and address.

Important items before you start the application:

• During the application process you will electronically sign the FFS Sales Contractor Agreement using DocuSign. DocuSign is the industry leader in Digital Transaction Management. Although DocuSign allows for easy access and retrieval of your signed Sales Contract Agreement, we suggest that you save and keep a printed copy of FFS Sales Contractor Agreement.  


• Your application will not be submitted to FFS until you submit payment. You can "Cancel" the application process at any time to stop the application process.  


• All information and signatures must be completed by the applicant. No other person can complete the information on behalf of the applicant.


• The system requires each applicant to have a unique email, therefore, all applicants are required to have their own email address.
